Shop Talk: Reporting serious incidents

​All serious incidents must be reported immediately to the Workplace Safety and Health Branch of the provincial government (WSH) and the scene of the incident must be preserved for WSH investigation. The incident must also be investigated by the workplace safety and health committee co-chairs (or their designates) to prevent the incident from happening again.
